NATIONWIDE NCMEC CUT-A-THON PARTICIPATION FORM-

We understand our proceeds must be turned in to Pivot Point by 7/1/10 in order to be eligible to receive the Crystal Service Award in July. Checks should be made out to NCMEC and mailed to Pivot Point Cares for Kids™, Attn: Heather Hodge, at 1560 Sherman Ave., Suite 700, Evanston, IL 60201.
